Our team works diligently to make sure you receive the maximum allowable benefit under your medical insurance plan. If you have questions about your account or to confirm whether we are an in-network provider with your insurance plan, we are here to help with any questions you may have.

Before Your Procedure

Please contact your insurance company prior to your scheduled surgery to determine if prior authorization is required. Failure to obtain the necessary approvals could result in a denial of coverage, making you financially responsible for the procedure.
Our billing office is available Monday through Friday from 8:00 a.m. to 5:00 p.m. For assistance or any inquiries, please call (712) 317-8402.

Payment Arrangements

  • If you do not have insurance or wish to set up a payment plan, please don’t hesitate to contact our billing office at the number provided above.
  • If your surgery is covered by Workers’ Compensation, we will need to gather all relevant information at the time your surgery is scheduled. This includes your Workers’ Compensation carrier details, such as their address, phone number, contact person, and an authorization number from your carrier.
